【PDCA日記 Vol. 1,086「薬価は誰が決めているのか？」】
私は健康オタクとして有名（？）ですが、何年かに一回は虫歯になってしまうことがあります。
私は3ヵ月に1回歯のクリーニングに行っているので、 3か月以上虫歯が放置されることはありません。
私のかかりつけの歯医者は話好きな人であり、アメリカの医療保険制度改革（オバマケア）の話題になることがあります。
アメリカの医療制度の特徴として、製薬会社が薬価を決めている点があります。
アメリカの医療費が高額になっているのは、政府ではなく製薬会社が薬価を決めていることも1つの要因です。
日本の場合、政府が薬価を決めているので、薬の値段が異常に高騰するということは今のところ起こっていません。
現役の日本の歯医者からすると、アメリカの医療保険制度の良い点も見えるらしく、歯の治療が終わってからも議論を続けてしまうことがあります。
日本で虫歯の治療をする場合、治療費は数千円ですみます。
アメリカで同じことをすれば、数万円近くかかる可能性があります。

日本政府が価格を決めている薬は例外的な存在です。
日本では国が価格を決めているものが、薬以外にもあります。
それは教科書代です。
薬と教科書代以外の商品・サービスについては、市場が価格を決めていることになりますね😊。
ただ、処方箋なしで購入可能な市販薬の場合、製薬会社が価格を決めており、広告を出すことも認められています。
アメリカのように、処方箋が必要な薬の価格を製薬会社が決め、広告を出せるようにしてしまうと値段が上がり過ぎて、本当に必要な人が薬を入手できなくなります。
資本主義の中でも、日本政府は処方箋薬を例外的な商品と考えていることになりますね😊。
＜Mr. PDCAのボンジュール英語「薬価」=「drug price」＞
今回出てきた「薬価」の英訳は、「drug price」になります。
「薬価は誰が決めているのか？」を英語にする場合、「Who decides the drug price?」とすればよいですね😊。
＜マドモアゼルPDCAのメルシー映画「雲のむこう、約束の場所」＞
本日は、映画「雲のむこう、約束の場所」を紹介します。
映画「雲のむこう、約束の場所」は、戦後、南北に分断された架空の日本が舞台です。
1996年、世界の半分を覆う共産国家群「ユニオン」は「エゾ」（北海道がモデル）を支配下に置き、島の中央にとほうもなく高い、純白の塔を建造しつつありました。
映画「雲のむこう、約束の場所」の世界では、青森県より南は日本が統治しており、津軽海峡ではユニオン軍と日米軍の衝突がしょっちゅう発生しています。
津軽半島に住む中学3年生の藤沢浩紀と白川拓也は、すぐ近くにある外国であるユニオンのエゾにそびえる塔にあこがれ、飛行機で国境の津軽海峡を越え、塔まで飛んで行く計画を立てていました。
この後のことは映画「雲のむこう、約束の場所」を鑑賞頂ければと思いますが、戦後、日本が南北に分断される可能性は結構高かったことが各種資料で確認されています。
第二次世界大戦後、日本はアメリカに占領されましたが、国土が分断されることはありませんでした。
当時、ソ連のスターリンは北海道の統治を考えていたと言われています。
アメリカがソ連の北海道侵攻を阻止したわけですが、日本の降伏タイミングが遅れていれば、映画「雲のむこう、約束の場所」や漫画「太陽の黙示録」の世界のように、日本が南北に分断される可能性は結構あったことになります。
漫画「太陽の黙示録」における日本の南北分断の原因は戦争ではなく、自然災害になっています。
漫画「太陽の黙示録」の世界では、福岡市に首都を置くサウスエリアで米ドルが流通し、アメリカに支配されています。
一方、ノースエリアでは札幌市が首都になっており、共産圏の影響を受けた行政機関が社会を運営しています。
安全保障を考える上で、漫画「太陽の黙示録」は参考になる資料です。

I'm famous as a healthy nerd, but once every few years, I sometimes have tooth decay.
I go to the tooth cleaning once every 3 months, so I don't leave my cavities for more than 3 months.

One of the characteristics of the medical system in the United States is that pharmaceutical companies set drug prices.
High medical costs in the United States are partly due to the fact that drug companies, rather than the government, determine drug prices.
In case of Japan, drug prices have been set by the government, so the price of drugs has not soared so far.
From the perspective of a Japanese dentist, it seems that the US medical insurance system has some good points.
If you want to treat dental caries in Japan, the cost of treatment is only tens of dollars.
Doing the same in the US can cost nearly tens of thousands of dollars.

P.S. In Japan, as a capitalist society, the prices of goods and services are to be determined by the market without government intervention.
Drugs priced by the Japanese government are exceptional.
In Japan, there are other things besides medicine that are priced by the country.
It's a textbook fee.
For products and services other than medicines and textbooks, the market decides the price 😊.
However, for over-the-counter drugs that can be purchased without a prescription, the drug company sets the price and is allowed to advertise.
As in the United States, if a pharmaceutical company decides the price of a drug that requires a prescription and allows it to be advertised, the price will rise too much and people who really need drugs will not be able to get them.
Even in capitalism, the Japanese government considers prescription drugs to be exceptional products 😊.
